Dr. Bream has extensive experience working in community based engagement in the Cobbs Creek community of Philadelphia, in rural communities in Alaska and Pennsylvania, and internationally in Guatemala. In particular, through close, productive, and trusting professional relationships with political, organizational, and social leaders, he has carried out community needs assessments, community participatory diabetes research and interventions, and high uptake COVID testing and vaccination programs. Through the Guatemalan collaborative he and his collaborators have developed student and community leader mobility to participate in local, national, and international stakeholder meetings. In addition to this work, he founded the Penn ACTIONetwork an interdisciplinary network of community based safety net providers, to integrate care for complex-needs patients between an academic health center and FQHC community sites. He works in project development and management engaging community representatives in the governance role, and founded the Dr. Bernett L. Johnson, Jr. Sayre Health Center, an FQHC in West Philadelphia, the United Community Clinics, the Guatemala Health Initiative in Guatemala, and recently the nascent African Immigrant Health program with students. Dr. Bream works with patient-led governance boards in FQHCs and works with local, state, and national primary care associations (NACHC, PACHC, HFP). He has had significant impact and has demonstrated success in forming trusting community relationships for research and clinical program development.
